2. Fax a Referral:
Download and complete our referral form, then fax it to 1-855-595-2926. Our office will follow up with the client upon receipt.

3. Patient Self-Scheduling:
You may also provide your client with our website link (www.unitedmentalwellness.com) so they can self-schedule a free 15-minute introductory call at their convenience.
