Option 1 – Share Our Website with Your Client
Direct your client to our website, www.unitedmentalwellness.com, so they can schedule a free 15-minute introductory phone call at their convenience. This gives them an opportunity to meet with our provider, ask questions, and see if our services are a good fit before committing to a full intake.

Option 2 – Submit Online
Complete the secure referral form below. This electronic form is embedded directly into our practice management system and allows for faster processing.

Option 3 – Print & Fax
